VA Patient Program Guide

Welcome to Your Community Care Program

Welcome to Momentum Medical Massage Therapy. We are pleased to support you as part of your VA Community Care treatment program.

Our program is designed to provide structured medical massage care with an emphasis on continuity of treatment, clinical progression, patient education, and recovery support.

Your VA authorization establishes a specific episode of care and treatment timeline. Our team will work with you to help organize your appointments, monitor treatment progress, and support completion of your authorized care within the applicable VA authorization period.

Treatment is provided by appointment only. Scheduling is coordinated according to clinical need, authorization timelines, provider availability, and continuity of care.

VA Authorization & Treatment Timeline

VA Community Care authorizations are time-sensitive and are generally structured within a defined episode of care, such as a 90-day or 180-day authorization period, as specified by the VA referral.

The treatment timeline begins according to the applicable VA authorization requirements. Patients are encouraged to maintain consistent scheduling so authorized visits can be completed within the approved period.

If remaining visits become difficult to complete because the authorization period is approaching expiration, Momentum Medical Massage Therapy may offer additional scheduling support when clinically and operationally available.

Our regular clinical scheduling is primarily available Monday, Tuesday, and Thursday. Limited Wednesday or Saturday appointments may be offered specifically when additional scheduling support is necessary to help a patient complete authorized care before expiration. These appointments are subject to provider availability and are not guaranteed.

Please remember that unused visits do not automatically extend a VA authorization. Momentum Medical Massage Therapy cannot independently extend or override an authorization expiration date.

Scheduling & Provider Availability

Momentum Medical Massage Therapy operates by appointment only. Our scheduling structure is designed to support continuity of care while providing flexibility for patients completing VA Community Care treatment programs.

Regular clinical scheduling is available Monday, Tuesday, Thursday, and Friday, generally between 9:00 AM and 4:00 PM, depending on provider availability.

Angelina Hall, LMT primarily provides care Monday, Tuesday, and Thursday during daytime clinical hours. Aaron Hall provides additional scheduling support, including selected appointments after 4:00 PM and Friday availability.

When clinically and operationally possible, earlier appointments or other scheduling accommodations may occasionally be arranged to help patients maintain continuity of care.

Wednesday and Saturday are reserved for limited scheduling support when necessary, particularly for patients approaching the end of their VA authorization period who need additional opportunities to complete authorized visits.

All appointment availability is subject to provider schedules, clinical workload, and treatment needs. Extended-hour, Wednesday, or Saturday appointments are supportive options and are not guaranteed regular appointment times.

Treatment Structure & Continuity of Care

Each treatment session is structured according to the patient’s current condition, clinical presentation, treatment response, and authorized plan of care.

Depending on clinical need, a session may include assessment, medical massage techniques, therapeutic manual intervention, soft-tissue or myofascial work, mobility and range-of-motion support, structural reassessment, nervous-system support, and therapeutic technology when appropriate.

Treatment methods may change throughout the program as the patient progresses. The goal is not to repeat the same treatment at every visit, but to respond appropriately to changes in pain, mobility, tissue restriction, inflammation, functional tolerance, and recovery.

To support continuity within the VA authorization timeline, patients may occasionally receive care from another qualified Momentum Medical Massage Therapy provider when appropriate. This may occur because of provider availability, scheduling needs, illness, emergencies, or the need to maintain treatment continuity.

Our providers work within the same clinical care structure so that treatment progression and relevant documentation can remain coordinated throughout the authorized program.

Patient safety, treatment quality, clinical appropriateness, and continuity of care remain the priority when determining treatment methods and scheduling.

Treatment Progress Reports & Patient Updates

Momentum Medical Massage Therapy provides structured progress updates throughout the VA Community Care treatment program so patients can better understand their treatment progression and response to care.

For a standard 12-visit treatment program, patient progress is generally summarized at three key stages:

• Visits 1–4: Initial treatment response and early clinical progression

• Visits 5–8: Mid-program progress, functional changes, and treatment development

• Visits 9–12: Overall treatment progression and end-of-program outcome review

These progress summaries may include changes in pain levels, mobility, range of motion, functional tolerance, sleep or recovery patterns, soft-tissue restrictions, treatment response, and other clinically relevant observations.

Patients will generally receive one consolidated progress update after each four-visit treatment phase rather than a separate detailed report after every appointment.

Routine appointment confirmations, scheduling updates, and other administrative communications are handled separately through Momentum Medical Massage Therapy’s communication and scheduling systems.

Clinical documentation for individual treatment visits continues to be maintained as appropriate. The patient progress summary is an additional communication tool intended to help patients understand how their care is progressing throughout the authorized treatment program.

Treatment outcomes vary by individual, and progress summaries document observed or patient-reported changes without guaranteeing a specific clinical outcome.

Patient Privacy, Technology & Communication

Momentum Medical Massage Therapy uses designated clinical and patient-support systems to help organize treatment information, appointment communication, clinical documentation, and continuity of care.

Our Momentum patient application and related clinical workflows are designed to support privacy-conscious handling of patient information and secure access to appropriate treatment-related functions.

Patients may receive appointment confirmations, reminders, scheduling updates, and other appropriate communications through Momentum’s designated systems.

Private medical information should be shared only through communication methods designated by the clinic. Patients should not send protected or sensitive health information through public social-media comments, posts, or other public channels.

Momentum Medical Massage Therapy also provides several general communication options, including clinic messaging, business phone or text, email, WhatsApp, and website contact options. The appropriate communication method may depend on the type of information being shared.

Educational resources, clinic information, aftercare guidance, movement and recovery education, and general patient-support content may be available through the official Momentum Medical Massage Therapy website and connected channels, including YouTube, Facebook, Instagram, and TikTok.

Social-media content is intended for general education and should not be used as a substitute for individualized medical advice or as a method for transmitting private health information.

Professional Boundaries & Patient Responsibilities

Momentum Medical Massage Therapy maintains a professional clinical healthcare-support environment. Respectful communication, appropriate conduct, and professional therapeutic boundaries are required throughout the treatment program. Patients are responsible for following clinic policies, communicating scheduling changes promptly, and participating appropriately in their continuity of care. Inappropriate behavior, harassment, unsafe conduct, repeated boundary violations, or repeated disregard for clinic policies may result in scheduling limitations or discontinuation of care when necessary for patient or provider safety. Patients are also encouraged to leave jewelry, rings, watches, and other valuables at home or securely stored before treatment. The clinic cannot assume responsibility for personal belongings that are lost, misplaced, or left behind.

Treatment Environment, Safety & Personnel Preparation

Momentum Medical Massage Therapy maintains a structured clinical treatment environment designed to support patient comfort, provider safety, therapeutic effectiveness, and continuity of care.

Room Temperature & Air Circulation: Treatment rooms are generally maintained within a comfortable clinical range, typically approximately 72–75°F. Air circulation, fans, and air-filtration equipment may be used as needed to support airflow and room conditions. We understand that individual temperature preferences vary; however, environmental equipment may need to remain in operation when necessary for appropriate room conditions and clinic operations. Please communicate significant temperature sensitivity or medically relevant concerns to your provider before treatment.

Sound & Treatment Environment: Low-volume music or therapeutic background sound may occasionally be used as part of the treatment environment. Patients with hearing sensitivity, sensory concerns, hearing impairment, or other relevant needs are encouraged to notify the provider so reasonable adjustments can be considered while maintaining an appropriate environment for neighboring medical offices and other patients.

Hearing Aids & Personal Devices: Hearing aids, earbuds, glasses, jewelry, watches, and similar personal items may need to be removed during certain treatments, including cranial or head/neck therapy. Patients are responsible for securing these belongings. Please inform your therapist if you require assistance communicating after removal of a hearing device.

Mobility Devices & Wheelchairs: Patients using wheelchairs, walkers, or other mobility equipment should notify the clinic before their first appointment so accessibility and treatment-room arrangements can be reviewed. Certain large or heavy powered mobility devices may not be suitable for all areas of the facility due to building access, space, or flooring limitations. Please contact the clinic in advance if special mobility accommodations are required.

Makeup, Skin Products & Treatment Linens: Treatment involving the face, head, neck, or cranial area may result in contact with treatment linens and face-cradle coverings. Patients wearing makeup, tinted products, heavy skincare products, or other transferable cosmetics should inform the therapist before treatment so protective coverings can be prepared when appropriate. This helps maintain sanitation standards and protects clinical linens and equipment.

Our goal is to make reasonable accommodations whenever possible while maintaining a safe, professional, sanitary, and functional treatment environment for every patient and provider.

Service Animals & Treatment Room Safety

Momentum Medical Massage Therapy welcomes service animals as required by applicable law. Service animals must remain under the handler’s control at all times and must not interfere with the safe delivery of treatment. If an animal becomes aggressive, threatening, disruptive, or is otherwise not under the handler’s control, treatment may be paused and the animal may be required to leave the premises in accordance with applicable law. For safety and treatment planning, patients are encouraged to notify us in advance when a service animal will accompany them. Pets and emotional support animals that do not qualify as service animals under applicable law are not permitted in the treatment area.

Late Arrival, Cancellation & Scheduling Responsibility

Appointment times are reserved specifically for each patient and are an important part of maintaining continuity within the VA authorization period.

Patients are encouraged to provide at least 24–48 hours’ notice whenever possible when requesting a cancellation, reschedule, or scheduling adjustment.

Patients arriving more than 15 minutes late may receive a shortened treatment session or may need to reschedule, depending on the remaining treatment time, provider availability, and clinical circumstances. Treatment time cannot automatically be extended when other patients are scheduled afterward.

Repeated late arrivals, missed appointments, last-minute cancellations, or prolonged gaps in scheduling may affect continuity of care and the ability to complete authorized visits before the VA authorization expires.

If a scheduling difficulty develops, please communicate with Momentum Medical Massage Therapy as early as possible. We will make reasonable efforts to help identify available scheduling options within the patient’s authorization timeline and provider availability.

Illness,Infection & Treatment Safety

Patient and provider safety are essential to maintaining continuity of care. Please notify Momentum Medical Massage Therapy before your appointment if you are experiencing fever, active infection, contagious illness, significant new symptoms, or another health change that may affect the safety or appropriateness of treatment.

Treatment may need to be modified, postponed, or rescheduled when a patient’s current condition creates a potential safety concern or when hands-on treatment may not be clinically appropriate.

Patients should also inform the provider of significant changes in medications, recent procedures or surgery, new injuries, physician restrictions, or other relevant changes in health status before treatment begins.

When appropriate, the provider may recommend that the patient contact their physician or other healthcare professional before continuing treatment. Momentum Medical Massage Therapy does not diagnose medical conditions and works within the professional scope of medical massage therapy.

Early communication helps us make appropriate treatment and scheduling decisions while protecting patients, providers, and others within the shared medical office environment.

Contact, Scheduling & Patient Support

Momentum Medical Massage Therapy provides ongoing communication and scheduling support throughout each patient’s authorized treatment program.

Patients may contact Momentum through the designated business communication methods listed on the official Momentum Medical Massage Therapy website, including messaging, business phone or text, email, WhatsApp, and other available private communication channels.

Appointment confirmations, reminders, scheduling updates, cancellations, and routine appointment communications may be provided through Momentum’s designated scheduling and patient-support systems.

Patients who are experiencing difficulty completing authorized visits within a 90-day or 180-day authorization period are encouraged to contact Momentum as early as possible. When scheduling capacity permits, additional appointment options may be offered to help support completion of authorized care. Limited Wednesday or Saturday appointments may be considered when clinically and operationally appropriate for patients approaching authorization expiration.

Scheduling accommodations are based on provider availability and cannot be guaranteed. Patients are encouraged to maintain consistent treatment scheduling throughout the authorization period rather than waiting until the final weeks of authorization whenever possible.

Momentum Medical Massage Therapy will continue to provide treatment updates, patient education, progress communication, and reasonable scheduling support with the goal of helping each patient participate successfully in their authorized treatment program.

Additional VA Patient Guidelines & Responsibilities

Makeup, Skin Products & Treatment Lines

Please inform your provider before treatment if you are wearing makeup, facial products, sunscreen, hair products, or other cosmetics that may transfer to treatment linens or equipment.

Certain treatments involving the face, head, neck, or cranial area may require direct contact with the skin. Protective face coverings or disposable barriers may be used when appropriate; however, some cosmetic products may still transfer or stain linens.

Patients are encouraged to arrive with minimal makeup when treatment of these areas is anticipated. Please discuss any skin sensitivity, recently applied topical medication, or product that should not be disturbed before treatment begins.

Momentum Medical Massage Therapy maintains treatment linens and equipment according to clinical hygiene procedures and appreciates patient cooperation in helping maintain a clean and safe treatment environment.

Check-in, Shared Medical Office & Front Desk

Momentum Medical Massage Therapy operates as a private clinical practice within a shared medical office environment. The building and common areas are used by multiple independent healthcare providers and practices.

Momentum does not maintain a dedicated front-desk receptionist or administrative assistant. Individuals working at reception counters, desks, or other common areas may belong to another medical practice and are not responsible for Momentum appointments, patient records, forms, scheduling, or clinical services.

Upon arrival, Momentum patients should follow the designated check-in process and use the provided iPad when instructed. Please do not request records, forms, appointment assistance, or other Momentum services from staff of neighboring practices.

Patients should arrive at the requested check-in time; however, treatment and clinical intake will begin according to the scheduled appointment time and provider availability. The provider will assist you with any required Momentum documentation and treatment preparation at the appropriate time.

Communication, Noise & hearing Assistance

Momentum Medical Massage Therapy operates within a shared professional medical office. Patients are asked to maintain a reasonable speaking volume in hallways, waiting areas, and treatment rooms to respect neighboring healthcare providers and their patients.

Please inform your provider if you have hearing difficulty or use hearing aids or other assistive devices. Hearing aids, eyeglasses, or similar items may need to be temporarily removed during cranial, head, neck, or facial treatment when necessary for safety and proper treatment access.

If a hearing device is removed, please let your provider know what communication method works best for you. We will make reasonable efforts to communicate clearly throughout the treatment.

Music, airflow, fans, lighting, and other treatment-room conditions are maintained to support a safe clinical environment. Please discuss comfort concerns with your provider rather than adjusting or turning off clinical or environmental equipment independently.

Program Completion, Final Review & Continue Care

As the authorized treatment program approaches completion, Momentum Medical Massage Therapy will review the patient’s overall treatment progression, functional response, remaining concerns, and relevant clinical observations.

For a standard 12-visit authorization, treatment progress is organized into three reporting phases: Visits 1–4, Visits 5–8, and Visits 9–12. A consolidated progress summary is generally provided following each four-visit phase.

At the completion of the authorized treatment period, a final clinical review may include changes in pain, mobility, range of motion, functional tolerance, sleep or recovery patterns, soft-tissue restrictions, treatment response, and other clinically relevant observations.

When continued care appears clinically appropriate, Momentum Medical Massage Therapy may prepare a Plan of Care or treatment summary to support communication regarding the patient’s continued treatment needs. Any additional VA Community Care authorization remains subject to VA review, referral requirements, and approval.

Completion of one authorization does not guarantee approval of another authorization. If a new authorization is issued, it will be treated as a new episode of care with its own authorization dates, approved visits, treatment progression, and documentation cycle.

Patient Education & Digital Resources

Momentum Medical Massage Therapy provides educational resources to support patients throughout their treatment and recovery process.

General educational information may include treatment preparation, aftercare, mobility and movement guidance, recovery support, body awareness, therapeutic techniques, and other information intended to help patients better understand and participate in their care.

Patients may access current clinic information and educational content through the official Momentum Medical Massage Therapy website and its connected platforms, including YouTube, Facebook, Instagram, and TikTok.

These resources are provided for general patient education and do not replace individualized medical evaluation, diagnosis, or recommendations from a physician or other qualified healthcare professional.

For privacy and security, patients should not post personal medical information, treatment details, or other protected health information through public social-media comments or posts.
